You ought to try dealing with Travelers Auto Insurance! After 25+ years of a flawless driving record, and 10 years with Travelers (30+ year with AAA which is a broker for Travelers) I had a small fender bender. When the claim was settled, my insurance rate increased 75% in one jump! Nauturally I objected and was treated to the most infuriating run-around. The result from everyone, including the Oregon Dpartment of Consumer and Business Services - Insurance Division, to which I made a formal complaint, was that no-one can interpret the codes that go into determining your auto insurance rate because it is all done by computer and there is some secret no-name company which collects all this inforamation on every consumer that puts all these points in the system which determines your rate - and of course is the proprietary and cannot be discussed. I think was infuriated me more was the total lack of reagard and down right rudeness of Travelers customer service people. I have never been subjected to such hostility. My final action - I am no longer insured with Travelers, and NEVER will be again!

Fantastic list! I actually had the pleasure of reading a letter that adhered perfectly to points 1-4 (5 wasn't applicable). The company in question had been wronged in the spirit of the contract, although not the letter, by the company I work for. It helps that I work someplace where management is reasonable and already knew the company had been wronged. But, it was still a fantastic letter. Not only did they get exactly what they requested but the letter was also show to our board as evidence for why we needed to take action.
